Temporary and permanent accessible parking tags are available to residents of the Town of Danby. Our office does not issue vehicle plates, please contact the DMV.
Medical certification is required, either using NY State DMV form 664.1, or with a signed note from a qualified practitioner on their letterhead. For more information please see the State DMV form instructions linked here: https://dmv.ny.gov/more-info/parking-for-people-with-disabilities
Most clinics have copies of the required form, but we do also have them here and are happy to print off a copy for you. Please bring a driver’s license or other State-issued ID of the person the application is for. If you are the parent/legal guardian of a disabled child, your ID is required. Please make sure that all fields in “Part 1” of the form are filled in and that you’ve signed and dated the form!
Temporary parking hang tags are red in color and valid up to six months based on the date of recovery from your practitioner. Notes written for dates after 6 months will be honored only up to the 6 month limit. After 6 months, please see your practitioner for a new note before applying for your new permit.
Permanent parking hang tags are blue in color and issued for up to five years. A new note is not required to renew a permanent (blue) handicap permit.
